Diabetes resources for Members
When you were diagnosed with diabetes, your doctor gave you advice on healthy steps to take and tests to get. But you may need additional advice or motivation as you work to manage your condition.
It’s important that you know where you can go for help. UPMC Health Plan offers members a number of diabetes resources, so you can decide what works best for you.
- Diabetes health coaching: This is the option to choose if you’d prefer one-on-one coaching over the phone. There’s no additional cost for speaking to a health coach. To connect with a health coach, call your doctor for a referral or contact a Health Care Concierge at 1-866-778-6073
- Diabetes self-management education: This option will allow you to get detailed information in a classroom setting from a certified diabetes educator. You’ll learn about blood glucose monitoring, the role of insulin, risk reduction, and healthy eating. A deductible and coinsurance may apply.
- Dietitian: Consider this option if you’d like a personalized meal plan to help control your diabetes. You’ll meet with a registered dietitian and discuss healthy eating options, recipes, and other topics. You may need to pay a deductible or copayment. To connect with a dietitian, call your doctor for a referral.
As a UPMC Health Plan member, you can also get a OneTouch Verio Flex blood glucose meter at no cost. You can connect the meter to your smartphone using the OneTouch Reveal app. Doing so will allow you to track your glucose levels and easily share data with your doctor.
